Depresión por aislamiento en el contexto de la pandemia y pospandemia: guía clínica para profesionales de la salud

| dc.description | Introduction: Isolation is an effective public health measure during infectious disease outbreaks. However, it is also a predictor of post-traumatic stress, depression, anxiety, among other conditions. Clinical practice guidelines represent a valid resource to address the negative effects associated with depression; nonetheless, these interventions must be adapted to the available resources and cultural context of each country. Methods: A panel of experts conducted a prioritization process covering four PICO (participants, intervention, comparison, and outcome) questions related to the management of pandemic-related isolation depression. A systematic search of clinical practice guidelines was performed across databases, meta-search engines, and institutional repositories of higher education. Additionally, a literature review was conducted in MEDLINE/PubMed, Web of Science, Cochrane Library, LILACS, and SciELO. The quality of the recommendations was assessed using the AGREE II instrument. Results: A set of clinical recommendations was developed based on the GRADE system. The guideline includes specific recommendations for the treatment of depression in adults in isolation contexts, such as those experienced during a pandemic. The main recommendations are: (1) initiate psychotherapy at the time of diagnosis, tailored to the patient’s individual needs; (2) prioritize cognitive-behavioral therapy (CBT) and interpersonal therapy (IPT) as first-line treatments; (3) in moderate to severe cases, consider combining psychotherapy with antidepressants; (4) incorporate psychoeducation as an integral component of the therapeutic approach; (5) in the presence of psychotic symptoms, prioritize pharmacotherapy; and (6) select the type and format of treatment according to resource availability, clinical risk level, and patient preferences. The guideline was validated as part of the study outcomes through a structured review process by clinicians, methodologists, and patients under controlled treatment and active follow-up. This validation represents a relevant finding of the study and supports its clinical applicability. Conclusion: The proposed CPG for Colombia holds theoretical value by adapting international evidence to the local context through the GRADE system, and practical value by providing clear recommendations for the management of depression in adults experiencing isolation. Key points include initiating psychotherapy at diagnosis, prioritizing CBT and IPT, combining with antidepressants in moderate to severe cases, and using pharmacotherapy in psychotic depression. This guideline offers useful, context-specific tools for clinical decision-making. | |
| dc.description.abstract | Introducción: El aislamiento es una medida de salud pública eficaz durante brotes infecciosos. Esta conducta es un predictor de estrés postraumático, depresión, ansiedad, entre otros. Las guías de práctica clínica representan un recurso válido para abordar los efectos negativos asociados a la depresión, estas intervenciones deben adaptarse a los recursos disponibles y la cultura de cada país. Metodología: Un panel de expertos realizó un proceso de priorización de tópicos que abordó cuatro preguntas PICO relacionadas con el manejo de la depresión por aislamiento pandémico. Se hizo una búsqueda sistemática de guías de práctica clínica en bases de datos, metabuscadores y repositorios institucionales de educación superior. Además, se efectuó una revisión de la literatura en MEDLINE/PubMed, Web of Science, Biblioteca Cochrane, LILACS y SciELO. La calidad de las recomendaciones se evaluó mediante la herramienta AGREE II. Resultados: Se elaboraron recomendaciones con base en el sistema GRADE. La guía fue validada por clínicos, metodólogos y pacientes con tratamiento controlado y en seguimiento. Conclusión: La GPC propuesta para Colombia tiene un valor teórico y práctico. Se emiten una serie de recomendaciones para el tratamiento de la depresión en adultos afectados por el aislamiento secundario al aislamiento propio de una pandemia. | |
